To disable form templates in PsychEase, the following steps should be taken:

1. Click Admin, then Map Forms To Folders

2.  A modal will display, containing a list of all form templates.  You can search for the form you would like to disable, or scroll through the list to find it.

3.  Once you have found the form you would like to disable, click the Edit (pencil) icon to the right.

4.  Click the checkbox in the Active column, to uncheck the box, then click the save (floppy disk) icon and click the Close button.  NOTE:  To cancel, click the arrow pointing to the left.

5.  That's all there is to it.  To verify it has been disabled, you can repeat steps 1 and 2 above to ensure the Active box is unchecked.
