If received this error, re-register the OCX file from CMD prompt.

There error received was 'Failed to load control 'fpCombo' from Flp32a30.ocx.  your version of Flp32a30.ocx may be outdated.  Make sure you are using the version of the control that was provided with your application.'

Go into the SysWOW folder located under Windows.  Shift + right click on file and copy path

Open CMD as Admin

Type in regsvr <insert file path>

Relaunch OT/QD
