When you add the Addiction Severity Index as an Outcome Measure for a patient and click Score, you get an error:
“Error setting up measure. Expected number of subscale entries in table OutcomeSubScales not found.”

To Reproduce the issue:
1. Go to Assessments…Outcome Measures
2. Click Add
3. Select Addiction Severity Index and click Ok
4. Click Score

Please contact DTI Technical Support so that your database can be updated to add the missing subscale entries.
The missing subscale entries has been corrected in QuicDoc V9.05.
