With the patient selected in the Patient window, select Nutrition Assessment from the Assessments menu.

Click on New

Enter or select all relevant information in the fields.

Click on Print, to print the report.

Click Ok to save.  The data will be saved and listed in the Nutrition Assessments window.

Click on Edit button or double click to open the assessment.
